This chart shows the dose as being still too high; it's still apparently causing Somogyi Rebound.
If the Caninsulin dose has been cut, it's apparently not by enough. Or perhaps Caninsulin isn't the best insulin for Rin Tin Toto. Dogs vary in their responses to different insulins.
Rin Tin Toto's veterinarian might decide to take an entirely different approach, and try him on one of the NPH insulins, such as Novolin-NPH, or Humulin-NPH. The veterinarian would probably also not make a direct conversion from the former 18 units of Caninsulin, but rather, drop the dose of biologically-active insulin by quite a bit.
An article that contains some information on dosing dogs and cats with NPH insulin, entitled Diabetes Mellitus in the Dog and Cat with Special Emphasis on Feline Diabetes, by Rhett Nichols, DVM, ACVIM, can be found on the Internet. It is a .PDF document It might be of some assistance to those wanting more information. It contains suggested starting doses, though the attending veterinarian is not likely to go, necessarily by a suggested starting dose, as Rin Tin Toto has already been receiving some kind of insulin for years. Rather, the veterinarian will take a look at the two recent curves, consider Rin Tin Toto's clinical signs or their absence, and also, adjust in light of any other known conditions Rin Tin Toto might have.
